$200  Initial Evaluation (90 minutes)

$125 Follow Up Visit (75 minutes…because it’s very challenging to complete
needed manual work combined with PRI movement training in a mere 60 mins)

I offer a discounted price for clients that do not need me to prepare documents for them to present
to their insurance company: $175 Initial Evaluation (90 mins) and $100 Follow Up Visit (75 mins)

  • Classical Physio is a fee-for-service clinic, & payment is expected at time of service.

  • Classical Physio is out-of-network with all health plans. Classical Physio will provide you with a copy of your bill (with the exception of Medicare - see below) that you can, at your discretion, submit to your health plan for reimbursement for the services your health plan covers. Reimbursement, by individual insurance providers, is based on very specific factors, and is not guaranteed.

  • You are responsible for contacting your insurance company to determine the nature of your insurance benefits and to obtain any necessary physician referrals and/or pre-authorizations for services.

  • Classical Physio has NO RELATIONSHIP with Medicare and does not accept Medicare benefits for services.

  • If you are a Medicare beneficiary, you understand that I am not enrolled as a Medicare provider.  Medicare has onerous technical and administrative requirements that must be met for services to be considered medically necessary covered benefits.  I believe those requirements take unnecessary time away from the services I provide, and some of my services are not covered by Medicare.  Thus, Classical Physio cannot submit claims to Medicare and Medicare will not pay for my services even though the same services might be paid by Medicare if you obtained them from a Medicare enrolled provider.  You, as a Medicare beneficiary, will also not be able to submit Classical Physio bills for reimbursement (as with other out-of network providers mentioned above).

  • Classical Physio is always happy to recommend a Participating Provider that accepts Medicare for PT services.

  • Accepted payments include cash, check and credit card.

  • Your visits may be payable with HSA account monies.  Check with your financial advisor.
